Lakeshore offers a comprehensive benefit package to our valued field employees; this coverage is provided by Century Healthcare Insurance. Employees become eligible to elect coverage when:

• You have worked two weeks
• You will still be on assignment on the first of the month following receipt of this notice
• You did not decline coverage enrollment during a prior enrollment window

You will be eligible to elect coverage after you have completed two weeks of work. Enrollment will occur on the first of the month following receipt of your election form. If you do not elect coverage you will not be able to elect coverage until our next open enrollment period unless you experience a ‘life changing event’ (i.e.: marriage, divorce, loss of other coverage, etc.)
